Medical Examiner's Certificate (DOT Medical Card)

The Medical Examiner's Certificate, commonly called the DOT medical card, is a federally required document certifying that a commercial motor vehicle driver meets the physical qualification standards established by the FMCSA under 49 CFR Part 391. The examination must be performed by a certified medical examiner listed on the FMCSA National Registry. A driver cannot legally operate a CMV in interstate commerce without a valid, unexpired medical card.

The DOT physical examination evaluates a driver's overall physical fitness to safely operate a commercial motor vehicle. The examination covers vision (at least 20/40 acuity in each eye), hearing (ability to perceive a forced whisper at five feet), blood pressure and cardiovascular health, musculoskeletal function, neurological health, and the absence of conditions that could impair driving ability such as uncontrolled epilepsy, insulin-dependent diabetes (without an exemption), or substance use disorders. The examiner also reviews the driver's medication history, as certain medications can disqualify a driver from operating a CMV.

The standard medical card is valid for up to 24 months, but drivers with certain monitored health conditions may receive certificates valid for only 12 months or less. For example, drivers with stage 1 hypertension (blood pressure between 140/90 and 159/99) are typically certified for one year, while drivers with blood pressure at or above 160/100 may receive only a temporary certificate. Drivers with diabetes who are granted a Federal Diabetes Exemption may also be subject to more frequent medical evaluations.

The medical examiner's certificate must be kept in the driver qualification file, and the driver must carry a copy while operating a CMV. When a medical card expires, the driver's CDL is automatically downgraded by the state DMV to a non-commercial status, which means the driver cannot legally operate a CMV until a new valid certificate is obtained. This automatic downgrade makes medical card expiration tracking critically important for fleet operations — a single missed renewal can ground a driver without warning and disrupt delivery schedules.
