Driver Qualification File (DQ File)
A Driver Qualification File is a mandatory personnel record that every motor carrier must maintain for each driver who operates a commercial motor vehicle. Required under 49 CFR Part 391, the DQ file must contain specific documents proving the driver is qualified and medically fit to operate a CMV. Missing or incomplete DQ files are among the most frequently cited violations during DOT audits.
The Driver Qualification File is the cornerstone of regulatory compliance for trucking companies. Under FMCSA regulations, every motor carrier must maintain a DQ file for each driver from the date of hire and retain it for three years after the driver leaves the company. The file must contain the driver's employment application (going back 10 years), a motor vehicle record (MVR) from every state where the driver held a license in the past three years, a road test certificate or equivalent, the annual review of driving record, a current medical examiner's certificate, and the driver's annual certification of violations.
The DQ file is a living document that requires ongoing maintenance. The annual MVR review must be conducted and documented every 12 months. The medical examiner's certificate must be current at all times — for most drivers this means renewal every 24 months, but drivers with certain health conditions may require annual or more frequent medical examinations. If any required document expires or is missing, the driver is technically unqualified to operate a CMV, and the carrier is in violation of federal regulations.
During a DOT audit, investigators review a sample of DQ files and assess whether each file is complete and current. A single missing document — such as an expired medical card or a missed annual MVR review — constitutes a violation that can contribute to a Conditional or Unsatisfactory safety rating. For fleets of any size, tracking the multiple expiration dates and renewal deadlines across all DQ files manually is error-prone.
